How to Use These Forms:


Online Reimbursement Form: To be used for reimbursement on expenses. The PTO does not reimburse for taxes. To ensure you aren't charged for taxes, print a copy of the Tax Exemption Certificate and turn it in to the business you are purchasing from. All​​ reimbursement ​​forms​ ​must ​​be ​​accompanied​ ​by ​​the​ sales receipts. These can be sent electronically to Treasurer, Christina Allen.


**If​ ​you​ ​have​ ​a​ ​large​ ​invoice​ ​for​ ​a​ ​standard​ ​Woodridge ​project​ ​such​ ​as​ ​Mule​ ​Market and Carnival,​ ​etc., our PTO Treasurer can​ ​pay​ ​the​ ​invoice​ ​for​ ​you​ ​instead​ ​of​ ​you​ ​paying​ ​first​ ​and​ ​being reimbursed. Remind​ ​the​ ​vendor​ ​that​ ​we​ ​are​ ​tax-exempt.​


Woodridge PTO Deposit​ ​Form​: to be printed and ​turned​ ​in with​ ​and​ ​Reimbursement​ ​Form​ ​which​ ​must​ ​be​ ​completed​ ​to​ ​deposit​ ​funds​ ​or​ ​to​ ​be​ ​reimbursed for​ ​expenses.​ ​​ ​The​ ​forms​ ​are​ ​a​ ​necessary​ ​part​ ​of​ ​our​ ​record​ ​keeping​ ​in​ ​case​ ​our​ ​PTO​ ​is​ ​ever audited.​ ​​ There is a Treasurer's Mailbox​ ​in​ ​the​ ​Teacher Workroom ​where​ ​you​ ​can​ ​pick​ ​up​ ​and​ ​drop​ ​off​ ​these​ ​forms​ ​as well.​ ​​ ​


Woodridge PTO Deposit Form​: To be used to deposit checks collected on behalf of the Woodridge PTO. 
All​ ​checks​ ​must​ ​be​ ​listed​ ​separately​ ​for​ ​each​ ​deposit,​ ​including​ ​name,​ ​check​ ​number​ ​and amount.
If​ ​you​ ​have​ ​a​ ​large​ ​deposit​ ​involving​ ​many​ ​checks,​ ​please email an​ ​Excel​ ​spreadsheet​ ​or​ ​Google Sheets​ ​file​ ​of​ ​the​ ​checks to our Treasurer, Christina Allen. Attach​ ​the​ ​deposit​ ​form​ ​to​ ​the​ ​top​ ​of​ ​the​ ​spreadsheet.
Cash​ ​can​ ​be​ ​combined​ ​into​ ​one​ ​lump​ ​sum.

Deposits ​less​ ​than​ ​$100​ ​should​ ​be​ ​dropped​ ​off​ ​in​ ​the​ ​PTO​ ​Mailbox​ ​in​ ​the Teacher Workroom. If you have​ ​turned​ ​in​ ​a​ ​deposit manually, please email PTO Treasurer, Christina Allen.


Tax Exemption Certificate: The Woodridge PTO ​cannot​ ​reimburse​ ​for​ ​sales​ ​tax​ ​​as​ ​we​ ​are​ ​a​ ​tax-exempt​ ​organization. When​ ​you​ ​are​ ​purchasing​ ​something​ ​for​ ​the​ ​Woodridge​ ​PTO,​ ​you​ ​may​ ​present​ ​the​ ​retailer​ ​with our​ ​tax-exempt​ ​certificate. If​ ​you​ ​pay​ ​sales​ ​tax,​ ​you​ ​will​ ​not​ ​be​ ​reimbursed,​ ​so​ ​please​ ​remember​ ​to​ ​have​ ​this​ ​certificate​ ​with you.​ ​​
